What is a Registration Form?
The Registration Form is a document which can be downloaded in PDF-Format after an application for eHerkenning is submitted. We require this form to be signed by the legal representative(s) registered for the organization you're applying for.
If additions are made to your eHerkenning, e.g. an extra authorization or an upgrade, a Registration Form is also generated. This is because additions or upgrades also require the legal representatives to sign for the application.
How to download the Registration Form
Depending on your situation, a registration form can be downloaded by logging onto My Reconi, or by logging on to the application itself. For both situations, the steps are provided below:
Logging onto My Reconi:
1. Please click here to log in.
2. After logging on, please select 'My Account' and 'My Requests'
3. A new screen will appear, in which you can click the pencil icon to open the application.
4. Upon opening the application, the Registration Form is available to download by clicking 'Download Request PDF'.
Logging onto your (new) eHerkenning application:
If your initial application for eHerkenning is pending, please follow the steps below:
1. Please logon to your application by clicking here.
2. After logging on, a page will appear in which you can click 'Download Request PDF' to download the Registration form.
Does your application number start with 016? In that case, the form is sent to the legal representative(s) or the Authorization Manager(s) by e-mail.
How to generate a Registration Form
A Registration Form is always generated based on the data entered in your application. You can generate a Registration Form by applying through our website or My Reconi. The form is available for download during this application process.
Signature requirements for the Registration Form
We kindly ask you to download, print and sign the Registration Form with an actual pen. Digital signatures are not allowed. In most cases, the form needs to be sent by traditional post. If not, it's mentioned on the form itself.
How to change faulty information on the form
The registration form is generated based on the details entered during the application process. If additional signatures are required, these can be placed on the form by writing the correct name on it. It's not possible to change the names on the form online.
Additionally, it's not allowed to change any details of the user, their authorizations or the organization details. on the Registration Form.
